Prefer a hard copy?

We prefer online nominations if possible, but we're happy to receive hard copy nominations as well.

There are a number of ways you can get a nomination form.

• Download from this page and print at home.
• Pick up a form from your nearest Customer and Library Service Centre (click here for locations and opening times).
• Call us on 5734 6200 to request a hard copy in the mail.

Completed forms can be returned by:

• emailing mitchell@mitchellshire.vic.gov.au
• dropping in to a Customer and Library Service Centre or
• mailing Mitchell Shire Council - Governance, Reply Paid 70358, Broadford VIC 3658.
