Maternal and Child Health Survey 2018

As part of Council's commitment to improving services and determining future needs for Maternal and Child Health Services, we are seeking feedback from parents and guardians. Your feedback will provide valuable input into the future direction of our Maternal and Child Health Service. 

The survey is open until 5pm Friday 14 December 2018.

A report, including your feedback, will be provided to our Public Health Department. Any opinions or information that could lead to identification of individuals will not be included. We are committed to ensuring your responses remain confidential, in accordance with the Information Privacy Act 2000. If you would prefer a hard copy survey, or have any questions, please contact the Public Health Coordinator on 03 5734 6253.
